Promotional Rate for the first 100 patients: $185 with documented qualifying diagnosis.

We offer assessments to help determine eligibility:

- PTSD Assessment:

- $115 if you are diagnosed with PTSD and require a provider-generated report.

- $50 if you are found not to have PTSD, as less time is required.

- Chronic Pain Assessment:

- $115 if you are diagnosed with chronic pain and require a provider-generated report.

- $50 if you do not meet the diagnosis criteria.

These assessments are separate from the medical marijuana certification process.

- A list of medications you are currently taking
